This is a suggestion about one of the errors cited; "urpme 
--auto-orphans" has always seemed unreliable, for me.

INTERNAL ERROR: hd0 has no mapping in device.map (when translating (hd0,5))

The kernel installation scripts (Mandriva 2011) have failed to get the 
kernel installed for me, recently, with this error being amongst the output.
After correcting  /boot/grub/device.map to have a proper mapping, kernel 
installation was successful.  It might be relevant to your problems with 
Grub.
